<h4>1 - What is fitness for purpose?&nbsp;</h4>The test meets the necessary performance criteria for a clinical binary examination to ensure it is suitable for making accurate and reliable clinical decisions in the intended patient population. Among others, performance criteria include clinical sensitivity and clinical specificity.&nbsp;<h4>2 - How to assess clinical performance?&nbsp;</h4>Assessing the clinical performance of a binary examination, stating results as 'positive' or 'negative,' involves calculating the examination's Clinical Sensitivity and Clinical Specificity. From the physician's point of view, clinical performance is evaluated by using predictive values, i.e., how likely is it that a positive or negative result corresponds to the presence or absence of the condition, which is also influenced by the prevalence of the condition in the population being tested.&nbsp;<h4>3 - What is Clinical Sensitivity?&nbsp;</h4>Clinical sensitivity (SS), also known as the True Positive Rate, measures the proportion of true positives correctly identified by the test. These intervals provide a range of values within which the true sensitivity or specificity is expected to lie with a certain level of confidence, typically 95 %. Please refer to the AQA:2021 for information about statistical models to compute the 95 % CI [1].<h4>6 - How to define targets?&nbsp;</h4>Target minimum values for clinical sensitivity and specificity should be identified. These values may originate from regulations or criteria based on the intended use of the binary results. A target or minimum value for the lower limit of the 95 % CI for result rate, i.e., SS or SP, (LLSS.95 or LLSP.95) or the upper limit of the 95 % CI for result rate (HLSS.95 or HLSS.95) should also be defined according to the purpose of the analysis. The target or minimum value (LLSStg or LLSPtg) is particularly significant when the impact of false results is critical. For example, for blood components used in transfusions, the screening for infectious diseases should be performed with tests associated with an LLSS.95 close to 100 %.&nbsp;Figure 2 illustrates five different tests, each assessed for clinical sensitivity and its associated uncertainty. These cases reflect different levels of compliance with a target sensitivity of 0.5. Case 1 is non-compliant, Cases 2 to 4 are partially compliant, and Case 5 is compliant. From a simplified statistical power view, the laboratory should always take into account the minimum number of samples when defining its target. This number can be estimated by simulation. For example, for a clinical sensitivity of 90 % the evaluation will require a minimum of 10 samples. It should be emphasized that samples must be representative of the target population for a clinical power evaluation - for example, epidemiologically representative samples of the types and subtypes of a given virological agent are required.<img src="https://storage.googleapis.com/labrulez-bucket-strapi-h3hsga3/Eurachem_Figure_2_Compliance_assessment_with_clinical_sensitivity_a21ece043c.jpg" alt="Eurachem: Figure 2. This presentation will review the laboratory perspective on the regulation and surveillance of approximately 270 pesticides and how this testing ties into federal government programs. Resulting methods used by the Ohio Department of Agriculture (ODA) Pesticide Residue laboratory show a lower quantitation limit at or below 10 parts per billion in complex matrices using a QuEChERS extraction paired with GC/MS/MS and LC/MS/MS. This work will summarize data found using these methods and compare goals, analysis requirements, and challenges between the USDA Agriculture Monitoring Service Pesticide Data Program and the U.S. EPA Region 5 with respect to pesticide use.
Additionally discussed will be strategies and challenges of using multiresidue methods on mass spectrometry platforms from the ODA including:

Deidre Damon has been a scientist and mass spectrometrist for the Ohio Department of Agriculture’s Consumer Protection laboratory since 2019. She earned her PhD in analytical chemistry at the Ohio State University, focusing on alternative sampling and ionization sources for mass spectrometry. Since 2021, she has supervised the Pesticide Residue and Industrial Hemp laboratories where she oversees regulatory and surveillance testing of pesticides in environmental and food commodities on behalf of the U.S. EPA, USDA, and FDA for the state of Ohio.

A Government Perspective on Pesticide Surveillance

GCMS is one of the most common analytical tools used by the forensic drug chemist. Just as there are different types of tools utilized by a skilled carpenter, there are specific GCMS tools and techniques employed to meet the needs of the forensic chemist. This presentation will illustrate the various GCMS tools and instrumentation to quickly screen unknown samples in under a minute, deconvolute coeluting peaks for spectral fidelity, identify unknowns based on low energy ionization spectra, and confidently quantitate target compounds in unknow matrices.

Kirk received his B.S. in Forensic Science and Chemistry from EKU and began his forensic tenure at the Miami Valley Regional Crime Laboratory in Dayton, Ohio and FDLE in the Orlando and Pensacola laboratories. Kirk left the forensic realm and began his career with HP/Agilent, working as a service engineer supporting the LC, GC, LCMS, GCMS, and ICPMS products. While working for HP Kirk earned his M.S. in Analytical Chemistry from MTSU. In 2005 Kirk left Agilent to attend the University of Cincinnati and earned his Ph.D. in Analytical Chemistry. Afterwards, he worked for the Midwest Research Institute (MRIGlobal) in Kansas City, as a Principal Chemist and Sr. Program Manager on DoD projects, staffing, designing, and building remote laboratories for placement throughout the world. In 2014, Kirk re-joined Agilent as a GCMS Applications Scientist focusing on forensic applications within the GCMS product line.

Increasing interest in microplastics in the environment has resulted in how microplastics may be regulated in this arena. These initiatives may occur at various levels and by various organizations, from international (e.g., European Chemical Agency, ISO documents) to national and subnational (e.g., federal and state efforts in the U.S.). They all require standardization of monitoring methods.
This presentation will provide an overview of the state of standardization, and challenges thereof, for microplastics measurement in the aquatic environment, from collection to extraction to quantification (e.g., by spectroscopic analysis). Particular emphasis is placed on efforts within the State of California, which is the first jurisdiction to require monitoring for microplastics.
